Financial Policies
1.
We
presently accept Medicare and Most PPO insurance plans. We do not
participate in any HMO plans.
2.
We
request that you make any co-pay or deductible payment at the time of
service.
3.
After
your visit we will bill Medicare and/or your insurance for you. After we
have received payment from insurance we will send you a statement from our
office for any existing balance.
4.
Accounts
delinquent over 90 days from the date of our statement will be sent to
collections.
5.
Payment
for cosmetic services is not covered by insurance and is expected at the
time of service.
